Dealing with Dumb

I work in a Universisty, and a student emailed me yesterday asking for account information. I told her that she will also need my signature, and asked if she could stop by on Monday. 

She brings me a blank form, and when I ask her to fill it out she seems confused. "Oh. You want me to fill it out before you sign it even?"

Yes. The answer to that is yes. 

I give her a pen and invite her to sit down. As she fills out the form she is asking questions that seem obvious.

Her: "Do I check the box that says 'Grant'?"
Me: "Is it for a grant?"
Her: "I dont' think so."
Me: "Then don't check the box."

Her: "What do I put for the due date?"
Me: "When do you need it?"
Her: "April 30th."
Me: "Then put April 30th."

She leaves. I'm perplexed. This student is an honors student and a one page form baffles her. I have two possiblities as to why this keeps happening. 
1. Everyone but me is really really dumb, and I am average
2. Everyone but me is average, and I am really, really smart
